Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist to provide in-house services at a Skilled Nursing Facility in Freeport, IL. This PRN position supports resident care and requires licensure as an SLP with CCC.

Responsibilities include evaluating language and swallowing disorders, developing treatment plans with measurable goals, documenting in NetHealth EMR, and coordinating care with facility staff. Competitive pay and opportunities to grow within the organization are offered.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC-SLP credential or eligibility.
• Active state licensure in practice locations and ongoing CE.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patients, families, and staff for carryover of therapy goals.
• Document compliant and medically necessary records per CMS, using NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screenings to meet needs timely and with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master\'s degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
