Job summary

Evolve Therapy Services, LLC in Silvis, IL is seeking a Speech Language Pathologist to join an in-house Skilled Nursing Facility team. You will deliver quality therapy, assess residents, and participate in treatment planning.

The role requires a Master’s in Speech Language Pathology, CCC, an active SLP license, and commitment to ongoing education. CMS-compliant documentation in NetHealth EMR will be part of daily duties, with opportunities for growth.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Active SLP license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities.
• Complete CMS-compliant documentation from CMS expectations in NetHealth EMR systems.
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs.
• Participate in screening protocols for establishing needs of residents are met timely and with clinical excellence

Education

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
