Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to provide in-house therapy at a Skilled Nursing Facility in Silvis, IL.

The ideal candidate will hold a Master's in Speech Language Pathology, CCC-SLP, and an active state license, with strong skills in evaluation, goal setting, and CMS-compliant documentation. Partnership-based role with full-time/part-time/PRN options.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists, short- and long-term goals.
• Instruct patient, family, and staff for carryover of therapeutic goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site program coordination.
• Screen residents to ensure needs are met promptly and with clinical excellence.

Job Position

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Full Time, Part Time, or PRN

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
