Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Freeport, IL to deliver comprehensive speech therapy and support residents’ communication and swallowing needs.

You will evaluate residents, develop targeted treatment plans, instruct patients and staff, and ensure CMS-compliant documentation in the NetHealth EMR system. This PRN role offers competitive pay and opportunities for growth within the organization.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC) required.
• Must have an active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services consistent with CMS and NetHealth EMR expectations.
• Evaluate residents and obtain data for treatment planning and implementation.
• Develop a treatment plan with target lists and short-/long-term goals.
• Instruct patients, families, and staff in speech-language procedures for daily carryover.
• Ensure CMS-compliant documentation in NetHealth EMR and maintain records.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
