Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist to provide in-house therapy services at our partner Skilled Nursing Facility in Hastings, PA. The role is PRN, focusing on evaluating, planning, and delivering evidence-based speech-language pathology care to residents.

Responsibilities include developing treatment plans, documenting in NetHealth EMR per CMS requirements, and guiding patients, families, and staff.

Qualifications

• Master's degree in Speech-Language Pathology.
• CCC or CCC-SLP certification and valid state licensure.
• Active license to practice as an SLP in the required state(s).
• Commitment to ongoing professional development and licensure maintenance.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards and productivity metrics.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols and coordination of site programs.
• Participate in resident screening protocols ensuring timely, clinical service.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
