Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility located in Torrington, CT. This PRN role focuses on providing high-quality speech therapy, evaluating residents, and developing treatment plans in a compliant setting.

The ideal candidate holds a Master's in Speech-Language Pathology, CCC, and an active license. Ongoing CEU are expected to maintain licensure, with CMS documentation performed in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Active SLP license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards
• Evaluate and obtain data for treatment planning and implementation
• Develop treatment plans with targets and goals
• Instruct patients, families, and staff to ensure carryover of therapy goals
• Complete CMS-compliant documentation in NetHealth EMR
• Oversee FEES/MBS protocols and site coordination
• Participate in screening protocols to meet resident needs in a timely manner

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
