Job summary

Evolve Therapy Services in New Haven, CT is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis. The role focuses on planning, delivering, and coordinating clinical speech-language pathology services to residents.

You will evaluate residents, develop treatment plans, document in NetHealth EMR per CMS guidelines, and collaborate with staff to ensure high-quality resident care. A Master's in SLP, CCC, and active state licensure are required.

Qualifications

• Master's degree in Speech-Language Pathology completed.
• CCC certification and state licensure active.
• Commitment to ongoing CEU and licensure maintenance.
• Ability to document CMS-compliant records.

Responsibilities

• Provide quality speech therapy services and initiate referrals as needed.
• Evaluate residents and collect data for treatment planning.
• Develop treatment plans with target list and goals.
• Educate patient, family and staff for carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in resident screening to identify needs.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
