Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to join an in-house Skilled Nursing Facility in Columbus, NE. This full-time role focuses on planning, organizing, developing, and directing speech-language pathology services in line with federal, state, and local standards.

You will evaluate residents, develop treatment plans with short- and long-term goals, instruct staff and families, and ensure CMS-compliant documentation in NetHealth EMR. Active SLP licensure is required.

Qualifications

• Master's degree in Speech-Language Pathology and CCC required.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning.
• Develop treatment plans with short-term and long-term goals.
• Instruct patient, family, and staff to support carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site program coordination.
• Screen residents to identify needs and ensure timely care.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
