Job summary

Evolve Therapy Services, LLC seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Central City, NE. This PRN role focuses on delivering high-quality SLP services, evaluating residents, and coordinating care with staff to support communication and swallowing health.

Qualifications

• Master's degree in Speech Language Pathology or equivalent.
• CCC certification or equivalent credential.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech-language therapy services in a SNF setting.
• Evaluate residents and collect data for treatment planning.
• Develop and implement treatment plans with short- and long-term goals.
• Educate patients, families, and staff on speech-language procedures.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site program coordination.
• Participate in resident screening protocols to ensure needs are met.

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
