Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist to join an in-house Skilled Nursing Facility in Glendale, WI. This PRN role focuses on planning, directing, and delivering speech-language pathology services to residents, with emphasis on compliance and quality care.

You will evaluate residents, develop treatment plans, educate staff and families, and maintain CMS-compliant documentation in NetHealth. Requires CCC and active WI SLP licensure, with ongoing CE credits.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with targets and short-/long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete compliant and medically necessary documentation per CMS in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.

Education

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:
