Job summary

Evolve Therapy Services is seeking a compassionate Speech-Language Pathologist to support an in-house Skilled Nursing Facility in South Range, Wisconsin. This PRN role focuses on delivering high-quality assessment and treatment to residents in collaboration with facility staff.

Responsibilities include planning individualized treatment, documenting results, and participating in FEES/MBS protocols where appropriate, while maintaining CMS-compliant records and progressing resident goals.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.

Responsibilities

• To provide quality speech therapy services and initiate referrals when appropriate.
• To evaluate and obtain data necessary for treatment planning and implementation.
• To develop a treatment plan with targets and goals.
• Instructs patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee protocols for FEES/MBS and coordinate site programs.
• Participate in screening protocols to meet resident needs with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
