Job summary

Evolve Therapy Services seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Minnesota. This full-time role (also offering part-time or PRN) focuses on delivering high-quality therapy, evaluating residents, and coordinating care with the interdisciplinary team.

Requirements include a Master’s in Speech Language Pathology, CCC-SLP, active Minnesota licensure, and commitment to ongoing professional development.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation.
• To develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities.
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems.
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs.
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education

Job Type

Full Time (offering $5,000 sign on bonus and tuition reimbursement!), Part Time, or PRN

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions
