Job summary

Evolve-Therapy-Services seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility. This full-time position offers a $5,000 sign-on bonus and tuition reimbursement for eligible staff, with part-time or PRN options based on need.

Responsibilities include planning and directing SLP services, treatment plan development, CMS-compliant documentation in NetHealth EMR, and coordination of FEES/MBS with site programs. Active SLP licensure and CCC are required.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Certificate of Clinical Competency (CCC) required.
• Active SLP license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Plan, organize, develop and direct speech-language pathology services.
• Provide quality speech therapy and initiate referrals when needed.
• Develop treatment plans with short-/long-term goals.
• Instruct patients, families and staff for carryover of therapy.
• Document CMS-required data in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Screen residents to identify needs and deliver clinical care.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time (offering $5,000 sign on bonus and tuition reimbursement!), Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
