Job summary

Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Franklin, MN. This full-time/part-time/PRN role offers a $5,000 sign-on bonus and tuition reimbursement, with benefits for full-time staff.

Responsibilities include planning and delivering SLP services, data collection for treatment, establishing goals, instructing patients and families, and maintaining CMS-compliant documentation in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC certification required.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services per standards and productivity requirements.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Educate patients, families, and staff for carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time (offering $5,000 sign on bonus and tuition reimbursement!), Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
