Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Franklin, MN. The position supports Full Time, Part Time, or PRN hours with a $5,000 sign-on bonus and tuition reimbursement options.

The therapist will plan, develop, and direct Speech Language Pathology services to ensure high-quality resident care. Applicants must hold a Master’s degree in Speech Language Pathology, a Certificate of Clinical Competency, and an active SLP license in

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC required for certification.
• Active SLP license in state(s) of practice is mandatory.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards and productivity.
• Evaluate and gather data for treatment planning and implementation.
• Develop a treatment plan with target, short-term and long-term goals.
• Instruct patients, families, and staff in speech-language procedures for daily carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to identify resident needs promptly and with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility Job Type: Full Time (offering $5,000 sign on bonus and tuition reimbursement!), Part Time, or PRN Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.


• To evaluate and obtain data necessary for treatment planning and implementation


• To develop a treatment plan that includes a target list, short-term, and long-term goals


• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities


• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems


• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs


• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• MUST have active SLP license in state(s) of practice.
