Job summary

Empath Health is seeking a compassionate PRN Speech-Language Pathologist to join our Home Health team in Hernando County, serving Spring Hill, Weeki Wachee, Brooksville, and nearby areas. You will conduct assessments, develop plans of care, and guide families on swallowing and communication strategies in the home setting.

This PRN role offers 5-15+ home visits weekly with potential to expand to part-time or full-time as we grow, emphasizing flexible scheduling and interdisciplinary collaboration

Qualifications

• Graduate of an approved/accredited college program that meets ASHA CCC requirements or is in process of accruing supervised experience for certification.
• Active Florida SLP license
• Minimum of 1 year of experience as a licensed SLP; home health care experience preferred

Responsibilities

• Perform initial and ongoing assessments including OASIS as appropriate to identify swallowing and communication ability.
• Develop a plan of care in consultation with physician and care team.
• Instruct patients/families on food consistency to maintain nutrition and prevent swallowing complications.
• Instruct patients/families on educational strategies to improve sensory-motor development, language and cognition.
• Prepare clinical/progress notes and submit timely.
• Collaborate with interdisciplinary team to ensure holistic care.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits available in Hernando County (including Spring Hill, Weeki Wachee, Brooksville, and surrounding areas). It is very flexible - you choose when & where you want to work!

Schedule: PRN role; 5-15+ home health visits per week. If desired, part-time or even full-time may be available as we grow!

What you'll Do

• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
• Develop a plan of care in consultation with the physician and other care team members.
• Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
• Instruct patients and families on using special educational strategies and techniques to improve sensory‑motor development, perception, memory, language, and cognition.
• Prepares clinical and progress notes of services provided, patient activities and condition and submit in a timely manner.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

What You'll Need

• Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
• Active Florida SLP license
• Minimum of 1 year of experience as a licensed Speech Language Pathologist (SLP); home health care experience preferred
