Job summary

Transition Home Healthcare is seeking an experienced Speech Language Pathologist to join our Houston, TX team on a PRN basis, serving North, Northeast, and Northwest service areas.

As a licensed SLP in Texas, you will implement speech therapy care plans, educate patients and families, and coordinate care with the multidisciplinary team while producing timely clinical notes. Home health experience and Spanish bilingual skills are a plus.

Qualifications

• Licensed Speech-Language Pathologist in Texas.
• Home health experience preferred.
• Bilingual in Spanish encouraged to apply.
• Experience with Home Care Home Base EMR is a plus.
• Knowledge of home health regulations and healthcare compliance.
• Ability to handle physical workload.
• Good driving record.
• Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Exceptional communication and people skills.

• Implements speech therapy care plans.
• Coordinates care with entire team.
• Educates patients and family members.

• Communicates effectively with patients, caregivers, healthcare providers, office support and other care team members.

Qualifications:

• Speech language pathologist in the State of Texas.
• Home health experience highly preferred.
• Bilingual (Fluent in Spanish) encouraged to apply.
• Experience with Home Care Home Base EMR (preferred but not required).
• Knowledge of home health regulations and general healthcare compliance.
• Ability to handle physical workload.
• Good driving record.
• Proficient level experience in Microsoft Office (Word, Excel, PowerPoint, Outlook).
