Job summary

Transition Home Healthcare is seeking an experienced Speech Language Pathologist for PRN shifts to serve Houston and surrounding areas. The role involves evaluating and treating patients in home health settings within North, Northeast, and Northwest service areas.

You will implement care plans, educate patients and families, coordinate with the care team, and document clinical notes. Texas licensure is required; bilingual Spanish is encouraged; home health experience is preferred.

Speech Language Pathologist

Shifts Available: PRN

Service Areas with Openings: North, Northeast, Northwest

Responsibilities:

• Facilitates rehabilitation of patients with emotional, physical, or mental disabilities.
• Implements speech therapy care plans.
• Coordinates care with entire team.
• Educates patients and family members.
• Prepares timely, complete, and accurate clinical notes.
• Communicates effectively with patients, caregivers, healthcare providers, office support and other care team members.

Qualifications:

• Speech language pathologist in the State of Texas.
• Home health experience highly preferred.
• Bilingual (Fluent in Spanish) encouraged to apply.
• Experience with Home Care Home Base EMR (preferred but not required).
• Knowledge of home health regulations and general healthcare compliance.
• Ability to handle physical workload.
• Good driving record.
• Proficient level experience in Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Exceptional communication and people skills.
