Job summary

All About Home Care is a Houston-based home health care agency seeking a Speech Language Pathologist to serve patients in the Greater Houston area. Experience in home health and ASHA-certified credentials are highly valued. Kinnser software is used for documenting patient care.

Ideal candidates will be licensed in Texas with preferred 2 years of experience in the field. Interested applicants should submit their resume for consideration.

Qualifications

• Must meet the education and experience requirements for ASHA CCC-SLP/CCC-A or be in the process of accumulating supervised experience.
• Currently licensed in the State of Texas.
• Two years of experience, preferred.

Responsibilities

• Implements and maximizes communication for patients.
• Aims to return individuals to optimum and productive living within patient capabilities.
• Participates in patient care planning with other health care personnel.
• Provides full range Speech Language Pathology services as ordered by the physician.
• Directs and supervises personnel as assigned.
• Takes initial history and conducts initial evaluation.
• Performs all skilled procedures in speech-language pathology.
• Consults with physicians and treatment teams.
• Writes progress reports to physicians regarding patient progress.
• Instructs patients and families in home programs.
• Presents in-services to staff.
• Documents evaluations, plans of treatment and goals for physician approval; writes progress notes each visit and reevaluation when duration ends; writes discharge summaries.
• Supervisory visits occur every two weeks for patients seen by home health aides.
• Assists physicians in evaluating level of function and revises care plans as needed.
• Prepares clinical and progress notes; advises and consults with family and other agency personnel.

We are currently looking for a Speech Language Pathologist / Speech Therapist to help serve patients in the Greater Houston area. If you have experience in home health, please submit your resume. Qualification:

• A person who meets the education and experience requirements for a certificate of Clinical Competency in Speech Pathology or Audiology granted by the American Speech-Language Hearing Association, or
• A person who meets the educational requirements for certification and is in the
• Process of accumulating the supervised experience required for certification.
• Currently licensed in the State of Texas.
• Two (2) years experience, preferred.
