Job description

BAYADA Home Health Care

Currently seeking an experienced Speech Language Pathologist (SLP) for a per diem opportunity providing home health visits at our Fayetteville, NC office.

Position details:

• Serving adult and geriatric patients in homes throughout Harnett County.
• Prior home care experience is a plus but not required.

As a home care SLP, you will be part of a multidisciplinary team offering rehabilitative care and skilled nursing, helping clients remain at home.

Responsibilities:

1. Make home visits within designated geographic areas.
2. Perform evaluations, diagnose, and provide therapy for speech, language, communication, or swallowing issues.
3. Develop and revise treatment plans based on evaluations and physician directives.
4. Contribute to multidisciplinary care planning.
5. Educate patients and families on rehabilitative activities to promote health and independence.
6. Document all patient interactions and interventions accurately using electronic records.

Qualifications:

1. Current North Carolina Speech Language Pathologist license.
2. Certificate of clinical competence from ASHA or equivalent educational and work experience, with certification exam pending or in progress.
3. Strong computer skills; experience with EMR preferred.
