Job summary

Granite VNA, New Hampshire’s largest home health and hospice provider, is seeking a compassionate Speech-Language Pathologist to join our home care team serving Laconia, Wolfeboro and Meredith. You’ll deliver individualized therapy to patients at home, focusing on communication, swallowing, cognition, safety, and quality of life.

Qualifications

• Master's degree in speech-language pathology from an approved program.
• CCC-SLP certification and NH licensure.
• Two years of inpatient/skilled/acute or community health SLP experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide evidence-based speech, language, cognitive, and swallowing therapy.
• Monitor progress and adjust treatment plans as needed.
• Educate patients and caregivers on treatment strategies and home programs.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians and the interdisciplinary care team.
• Participate in care conferences, quality initiatives, and educational programs.
• Travel safely throughout the Granite VNA service area to provide care in patients’ homes.

JOB SPECIFICATIONS:

Schedule: Monday–Friday | 8:00 a.m.–4:00 p.m.

Location: Laconia, Wolfeboro and Meredith areas (Lakes Region)

Pay: $38-$56/hour (depending on experience) + Mileage Reimbursement at the Federal Rate

Requirements

1. Master's degree in speech-language pathology, communications sciences and disorders, or communications disorders.
2. Graduate of an approved speech-language program (ASHA) with a certificate of clinical competency in speech-language pathology (CCC-SLP), accompanied by current New Hampshire speech-language pathology license.
3. Two years of inpatient/skilled/acute or community health speech-language experience.
4. Valid driver's license, reliable transportation and an acceptable driving record.
