Aura Care Group is seeking a Speech Therapist (SLP) to evaluate and treat communication, cognitive and swallowing disorders in our residents. You will assess needs, develop individualized plans and provide therapy to improve speech, language, memory, and swallowing safety.

The role requires Minnesota licensure, a master’s degree in Speech-Language Pathology, and preferably CCC-SLP. Strong documentation, communication and collaboration with the care team are essential to help residents achieve

Qualifications

• Current and active SLP license in Minnesota (or eligibility).
• Master’s degree in Speech-Language Pathology from an accredited program.
• Certificate of Clinical Competence (CCC-SLP) from ASHA preferred.
• Strong assessment, treatment, and documentation skills.
• Excellent communication and interpersonal skills.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsabilités

• Evaluate residents/patients for communication, cognitive, and swallowing needs.
• Develop and implement individualized treatment plans based on assessments and physician orders.
• Provide therapy services to improve speech, language, voice, fluency, memory, problem-solving, and swallowing safety.
• Document evaluations, progress notes, and discharge summaries in compliance with professional, state, and federal standards.
• Educate residents, families, and staff on communication strategies, swallowing precautions, and home programs.
• Collaborate with physicians, nursing, and other therapy disciplines to ensure coordinated care.
• Participate in interdisciplinary care plan meetings and quality improvement programs.
• Maintain professional licensure and adhere to ethical practice standards.
