Job summary

Las Colinas Post Acute in Ontario, CA is seeking a compassionate Speech-Language Pathologist (SLP) or Clinical Fellow (CFY) to join our rehabilitation team.

You will provide speech, language, communication, and swallowing therapy to residents, develop individualized treatment plans, and collaborate with nursing and therapy colleagues in a supportive, resident-centered care environment. CFYs receive supervision to complete the Clinical Fellowship.

Qualifications

• Valid California SLP license or eligibility to practice as a Clinical Fellow.
• CPR certification.
• Skilled nursing or post-acute experience preferred.
• Strong communication, organization, and interpersonal skills.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Provide speech, language, communication, and swallowing therapy services to residents.
• Develop and implement individualized treatment plans.
• Evaluate resident progress and adjust treatment plans as appropriate.
• Collaborate with nursing, therapy, and other interdisciplinary team members.
• Complete timely and accurate documentation using NetHealth and PointClickCare.
• Educate residents, families, and care teams as appropriate.

Schedule: Full-Time or Per Diem

Pay Range: $48-$52 per hour, based on experience and employment status

Las Colinas Post Acute is looking for a compassionate and motivated Speech-Language Pathologist (SLP) or Clinical Fellow (CFY) to join our rehabilitation team.

Whether you're an experienced SLP or beginning your career as a Clinical Fellow, this is an opportunity to make a meaningful impact while working alongside a collaborative and supportive team.

Responsibilities

• Provide speech, language, communication, and swallowing therapy services to residents
• Develop and implement individualized treatment plans
