Job summary

Las Colinas Post Acute in Ontario, CA is seeking a dedicated Speech-Language Pathologist (SLP) or Clinical Fellow (CFY) to join our rehabilitation team. This full-time or per diem position offers a competitive hourly pay range and a supportive, collaborative environment for growth.

Qualifications

• Valid California SLP license or eligibility to practice as a Clinical Fellow.
• CPR certification.
• Skilled nursing or post-acute experience preferred.
• Strong communication, organization, and interpersonal skills.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Provide speech, language, communication, and swallowing therapy services to residents.
• Develop and implement individualized treatment plans.
• Evaluate resident progress and adjust treatment plans as appropriate.
• Collaborate with nursing, therapy, and other interdisciplinary team members.
• Complete timely and accurate documentation using NetHealth and PointClickCare.
• Educate residents, families, and care teams as appropriate.
