Job summary

Las Colinas Post Acute in Ontario, CA is seeking a compassionate Speech-Language Pathologist (SLP) or Clinical Fellow (CFY) to join our rehabilitation team. This role offers full-time or per diem schedules with competitive hourly wages and benefits through PACS Group.

You will provide therapy to residents, develop individualized plans, and collaborate with a supportive interdisciplinary team in a post-acute setting.

Qualifications

• Valid California SLP license or eligibility to practice as a Clinical Fellow.
• CPR certification required.
• Skilled nursing or post-acute experience preferred.
• Strong communication, organization, and interpersonal skills. Commitment to resident-centered care.

Responsibilities

• Provide speech, language, communication, and swallowing therapy services to residents.
• Develop and implement individualized treatment plans.
• Evaluate resident progress and adjust treatment plans as appropriate.
• Collaborate with nursing, therapy, and other interdisciplinary team members.
• Complete timely and accurate documentation using NetHealth and PointClickCare.
• Educate residents, families, and care teams as appropriate.

Speech-Language Pathologist (SLP) / Clinical Fellow (CFY) Las Colinas Post Acute – Ontario, CA Schedule: Full-Time or Per Diem Pay Range: $48–$52 per hour, based on experience and employment status

Las Colinas Post Acute is looking for a compassionate and motivated Speech-Language Pathologist (SLP) or Clinical Fellow (CFY) to join our rehabilitation team.
