Job summary

Centra Healthcare Solutions is seeking a Speech Language Pathologist (SLP) for a contract assignment to assess and treat patients with speech, language, and swallowing needs in a SNF/LTC setting.

The role involves collaborating with an interdisciplinary team, following physician orders, and setting achievable goals to restore communication and safe swallowing for patients’ daily activities.

Qualifications

• Graduate of an accredited Speech Language Pathology program.
• Current state license as a Speech Language Pathologist / SLP or actively pursuing licensure.
• CPR certification.
• SLP-CCC certification.

Responsibilities

• Assess referred patients requiring treatment for speech, hearing, and/or language deficits.
• Teach patients how to produce sounds, improve voice quality, and enhance oral or written language skills.
• Instruct patients in strengthening oral muscles or using compensatory strategies for safe swallowing.
• Work within an interdisciplinary team to develop individualized plans of care.
• Provide skilled speech therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet goals of speech therapy interventions.
• Set realistic and achievable goals to restore reliable communication and swallowing skills for patients’ educational, vocational, and social roles.
