Job summary

Centra Healthcare Solutions Inc. seeks an experienced Speech Language Pathologist (SLP) for a contract assignment in a SNF/LTC setting. The role requires CCC credentials and licensure in the applicable state, working with an interdisciplinary team to deliver targeted speech and swallowing therapies.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or in the process of licensure.
• SLP-CCC credential required.
• Ability to work within an interdisciplinary team.
• Provide skilled speech therapy services per physician orders.

Responsibilities

• Develop a plan of care with the interdisciplinary team.
• Provide speech therapy services/interventions as ordered by physicians.
• Assess patient needs and modify care to meet goals of speech therapy.
• Set realistic goals for communication and swallowing to support education, work, and social participation.

A great SNF / LTC is looking for an experienced Speech Language Pathologist / SLP to join their growing team for a contract assignment! This is a contract position. Days schedule.

This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills so that the patient can fulfill their educational, vocational and social roles.
• Candidate must be an SLP-CCC.
