Job summary

Centra Healthcare Solutions Inc. is seeking an experienced Speech Language Pathologist (SLP) for a contract assignment in a SNF/LTC setting.

The role requires collaborating with an interdisciplinary team, delivering skilled therapy per orders, and developing measurable plans of care to improve communication and swallowing abilities. Candidate must hold an SLP-CCC and be licensed or in process for state licensure.

Qualifications

• Must have graduated from an accredited program.
• Current state license (or in process) as SLP/SLP.
• Collaborate with an interdisciplinary team to develop a plan of care.
• Provide skilled speech therapy per physician orders.
• Assess needs, plan, evaluate and modify care for speech therapy goals.
• Set realistic goals for communication and swallowing.

Responsibilities

• Deliver speech therapy services per physician orders.
• Collaborate with care team to develop and monitor plan of care.
• Assess patients and adjust interventions to achieve goals.
• Ensure patient progress toward communication and swallowing objectives.

A great SNF / LTC is looking for an experienced Speech Language Pathologist / SLP to join their growing team for a contract assignment! This is a contract position. Days schedule.

This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills so that the patient can fulfill their educational, vocational and social roles.
• Candidate must be an SLP-CCC.
