Springfield Center for Living and Rehabilitation in Springfield, Vermont is seeking a dedicated Part-Time Speech-Language Pathologist (SLP‑CCC) to join our in‑house rehabilitation team in a skilled nursing and long‑term care setting.
As part of an interdisciplinary team, you will evaluate and treat residents, develop individualized plans, and document using Net Health and PCC. A Vermont license and CCC‑SLP are required, with SNF/rehab experience preferred.

Utilize Net Health and PointClickCare (PCC) documentation systems
Evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive‑communication, and swallowing disorders
Provide evidence‑based therapeutic interventions to maximize functional outcomes and quality of life
Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and family members
Complete timely and accurate documentation in Net Health and PCC
Participate in care planning meetings and interdisciplinary team discussions
Ensure compliance with all state, federal, and facility regulations
Master's Degree in Speech‑Language Pathology
Current Vermont Speech‑Language Pathology license
Certificate of Clinical Competence (CCC‑SLP) required
Skilled nursing, rehabilitation, or long‑term care experience preferred
Strong clinical, communication, and organizational skills
Commitment to providing high‑quality, resident‑centered care
