Job summary

Springfield Center for Living and Rehabilitation in Springfield, Vermont is seeking a dedicated Part-Time Speech-Language Pathologist (SLP‑CCC) to join our in‑house rehabilitation team in a skilled nursing and long‑term care setting.

As part of an interdisciplinary team, you will evaluate and treat residents, develop individualized plans, and document using Net Health and PCC. A Vermont license and CCC‑SLP are required, with SNF/rehab experience preferred.

Qualifications

• Master's Degree in Speech-Language Pathology required.
• Current Vermont Speech-Language Pathology license required.
• Certificate of Clinical Competence (CCC‑SLP) required.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsibilities

• Evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive‑communication, and swallowing disorders.
• Provide evidence‑based therapeutic interventions to maximize functional outcomes and quality of life.
• Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and family members.
• Complete timely and accurate documentation in Net Health and PCC.
• Participate in care planning meetings and interdisciplinary team discussions.
• Ensure compliance with state, federal, and facility regulations.
