As a Speech-Language Pathologist, you will:
Assess residents who require speech, language, cognitive, or swallowing rehabilitation services
Provide direct patient care based on each resident’s medical condition, functional abilities, limitations, and rehabilitation potential
Develop and implement individualized treatment programs with realistic and achievable goals
Monitor resident progress and modify treatment plans as appropriate
Complete evaluations, progress notes, recertifications, and discharge documentation
Educate residents, families, and staff on appropriate therapeutic techniques and strategies
Collaborate with nursing, therapy, and other interdisciplinary team members to provide comprehensive, resident-centered care
Maintain accurate and timely documentation
Ensure compliance with confidentiality, HIPAA, Residents’ Rights, and applicable state and federal regulations
Perform other duties as assigned by the Director of Rehabilitation
Graduate of an accredited Speech-Language Pathology program
Current and valid Vermont SLP license in good standing
Strong communication, organizational, and interpersonal skills
Experience in long-term care and/or geriatric care preferred, but not required
Compassionate, dependable, and committed to providing high-quality resident care
Ability to work effectively as part of an interdisciplinary team
