Job summary

Evolve Therapy Services, LLC seeks a Speech Language Pathologist to support an in-house Skilled Nursing Facility in Blackwell, WI. We offer Full Time, Part Time, or PRN schedules to fit your needs and a collaborative care environment.

You will plan, develop, and direct Speech Language Pathology services, conduct assessments, create treatment plans with short- and long-term goals, supervise documentation in NetHealth, and coordinate FEES/MBS protocols with the care team to ensure CMS-compliant

Qualifications

• Master’s degree in Speech Language Pathology required.
• CCC credential required.
• Active SLP license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover of goals in daily activities.
• Complete compliant CMS-mandated documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols and coordination of site programs.
• Participate in screening protocols to identify resident needs and ensure timely care.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions could include:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
