Job summary

Evolve Therapy Services, LLC is seeking a compassionate Speech Language Pathologist to serve residents at our in-house Skilled Nursing Facility in Fort Atkinson, Wisconsin. This part-time or PRN role focuses on delivering high-quality evaluation, treatment, and documentation in accordance with CMS expectations.

The position requires a Master’s in Speech-Language Pathology, CCC, and an active state license, with opportunities for professional growth and competitive compensation based on part-time

Qualifications

• Master's degree in Speech-Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning.
• Develop treatment plans with short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures to carry over remediation goals.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Part Time or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
