Job summary

Evolve Therapy Services, LLC in Antigo, WI, seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility to oversee speech-language services and ensure high‑quality resident care.

The ideal candidate will deliver therapy, collect data for treatment plans, and develop short- and long-term goals while educating residents, families, and staff. CMS documentation is done in NetHealth EMR and FEES/MBS protocols are coordinated at the site.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC (Certificate of Clinical Competency) required.
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services to residents.
• Evaluate data for treatment planning and implementation.
• Develop treatment plans with target, short‑ and long‑term goals.
• Educate patient, family and staff for carryover in daily activities.
• Maintain CMS-compliant documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols with site coordination.
• Participate in screening to identify resident needs timely and effectively.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions could include:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
