Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to provide in-house services at a Skilled Nursing Facility in Rice Lake, WI. This PRN position focuses on delivering high-quality speech therapy and ensuring CMS-compliant documentation.

Responsibilities include evaluating patients, developing treatment plans with goals, guiding families and staff, and coordinating FEES/MBS protocols. Must hold CCC-SLP and active state license.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC-SLP).
• Active licensed SLP in states of practice.
• Commitment to ongoing professional development to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target list, short- and long-term goals.
• Instruct patient, family, and staff for carryover to daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in screening to meet resident needs timely and with clinical excellence.

Education

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
