Job summary

Sunnyvale Post Acute is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team in Sunnyvale, CA. We offer Full-Time, Part-Time, and PRN opportunities with pay rate of $60.00–$65.00 per hour, DOE.

You will evaluate residents, provide individualized therapy for communication, cognitive, and swallowing needs, and collaborate with an interdisciplinary team to improve residents' quality of life. New grads welcome.

Qualifications

• Current California Speech-Language Pathologist (SLP) license.
• Experience in skilled nursing, post-acute care, or rehabilitation preferred.
• Strong communication, clinical, and organizational skills.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.
• Provide skilled therapy services based on resident needs and care plans.
• Monitor resident progress and modify treatment approaches as appropriate.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, therapy, dietary, and other interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment recommendations and strategies.
• Promote resident safety, independence, communication, and quality of life.
• Maintain compliance with applicable professional, state, and federal standards.
