Job summary

Sigma Health Rehab in Utica, NY is seeking a full-time Speech Language Pathologist (SLP) to work in a Skilled Nursing Facility. You will administer evaluations, develop treatment plans, monitor progress, and document outcomes to help residents regain independence.

Qualifications

• A graduate of an accredited SLP program.
• Current New York State SLP License.
• Experience in a skilled nursing or geriatric care setting is preferred, but new graduates with relevant experience are encouraged to apply.

Responsibilities

• Administer SLP evaluations to diagnose residents and develop treatment plans.
• Monitor residents' progress and adjust treatments accordingly.
• Record initial evaluations, treatment progress, and discharge information.
• Observe progress and prepare detailed reports.
• Maintain confidentiality of resident information.
• Ensure compliance with applicable laws, regulations, or standards.
• Maintain cooperative relationships within the healthcare team.

Key Responsibilities:

• Administer Speech Therapy evaluations & examinations to diagnose residents & develop treatment plans.
• Monitor residents' progress and adjust treatments accordingly.
• Record information on the initial evaluation, treatment, progress, and discharge of clients.
• Observe and assess the progress & prepare detailed reports.
• Maintain resident confidence by monitoring confidential information processing.
• Ensure compliance with relevant laws, regulations, or standards.
• Maintains a cooperative relationship among healthcare team members
