Job summary

Topaz Fiscal Services LLC is seeking a Speech Therapist to join our rehabilitation team in Florida. You will assess referred patients and provide direct care to evaluate medical conditions, functional abilities, and rehabilitation potential, under physician direction.

You will establish treatment programs with realistic goals, document patient progress, and educate patients and families in suitable therapy methods.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must hold a valid professional license and registration for state of NY.

Responsibilities

• Assess referred patients for rehabilitation needs.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and rehabilitation potential.
• Establish and administer treatment programs with goals based on patient capacity and tolerance under physician direction.
• Set realistic, achievable goals for patients.
• Document and record patients' conditions.
• Educate patients and families in appropriate therapy methods.
• Complete evaluations, weekly progress notes, recertifications and discharge documentation.
• Collaborate with all disciplines to deliver patient-centered care.
• Perform other duties as assigned by the Director of Rehabilitation.

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients' condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long-term care and/or geriatric care is preferred but not limited to.
