Job summary

Lafayette Nursing and Rehabilitation Center is seeking a Speech Therapist to assess and treat patients in a geriatric rehabilitation setting. You will develop and monitor individualized therapy programs, document progress, and educate patients and families.

Responsibilities include goal-directed treatment, collaboration with the care team, and discharge planning under the Director of Rehabilitation. Knowledge of HIPAA and confidentiality is essential.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state NY
• Experience with long-term care and/or geriatric care preferred

Responsibilities

• Assess referred patients needing rehabilitation.
• Provide direct patient care to assess medical condition and function.
• Establish and administer treatment programs with goals.
• Set realistic goals based on capacity and tolerance.
• Document patient status and progress.
• Educate patients and families in therapy methods.
• Prepare evaluations, progress notes, recertifications and discharge docs.
• Collaborate with all disciplines for patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Ensure confidentiality and HIPAA compliance.

Job description

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
