Job summary

Topaz Fiscal Services seeks a Speech Therapist to assess referred patients requiring rehabilitation and to deliver direct care under physician direction in a routine long-term care setting.

The role emphasizes evaluating functional capacity, documenting progress, and educating patients and families on therapy approaches, while collaborating with interdisciplinary teams to ensure optimal patient-centered care.

Qualifications

• Graduate from an accredited speech therapy program.
• Must hold a valid professional license and NY state registration.
• Experience in long-term care or geriatric settings preferred.

Responsibilities

• Assess referred patients to determine rehabilitation needs.
• Provide direct patient care to evaluate medical condition and rehab potential.
• Develop and administer treatment plans under physician direction.
• Set achievable goals aligned with patient capacity.
• Document patient status, progress, and outcomes.
• Educate patients and families on therapy methods.
• Collaborate with disciplines to deliver patient-centered care.
• Perform additional duties assigned by the Director of Rehabilitation.
• Ensure confidentiality and adherence to patient rights.

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long-term care and/or geriatric care is preferred but not limited to.
