Job summary

Homeside Rehab is actively hiring Speech-Language Pathologists (SLPs) to provide individualized, evidence-based therapy in patients’ homes across the Hudson Valley, NY. You’ll help improve communication, cognition, feeding, and safe swallowing while enjoying a flexible schedule and supportive clinical team.

This role requires NYS SLP licensure and at least 1 year of relevant experience; responsibilities include evaluations, treatment planning, documentation, and collaboration with the care team

Qualifications

Active New York State SLP license required.
Minimum 1 year of home health, outpatient, EI, or SNF experience preferred.
Excellent clinical assessment and communication skills.
Ability to work independently in a home health setting.

Responsibilities

Perform comprehensive speech-language evaluations and create individualized treatment plans.
Provide evidence-based treatment for speech, language, articulation, fluency, voice, cognitive-communication, and swallowing when applicable.
Deliver skilled SLP services in patients' homes enabling functional independence.
Complete accurate, timely electronic documentation.
Collaborate with physicians, caregivers, and the interdisciplinary team.
Educate patients and caregivers on home programs and communication strategies.
Communicate effectively with office staff about scheduling and progress.

Homeside Rehab is seeking compassionate and dedicated Speech-Language Pathologists (SLPs) to provide individualized, evidence-based speech therapy services to patients in their homes throughout the Hudson Valley, New York.

Responsibilities

Perform comprehensive speech-language evaluations and develop individualized treatment plans based on each patient's needs and goals.
Provide evidence-based treatment for speech, language, articulation, fluency, voice, cognitive-communication, and swallowing disorders, when applicable.
Deliver skilled speech therapy services in patients' homes while promoting improved communication and functional independence.
Complete accurate, timely, and compliant electronic documentation.
Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.
Educate patients and caregivers on home programs and communication strategies.
Communicate effectively with office staff regarding scheduling, patient progress, and clinical needs.

Qualifications

Active New York State Speech-Language Pathologist License (Required)
Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred
Excellent clinical assessment and communication skills
Ability to work independently in a home health setting
Basic computer proficiency for electronic documentation
Valid New York State driver's license and reliable transportation preferred
