Job description

• Responsibilities include: Developing and implementing individualized treatment plans to address various diseases and impairments, ensuring a positive rehab experience for patients and their families.
• Work environment: Supportive rehab setting utilizing progressive treatment modalities with in-house therapy and local management support.
• Qualifications: Graduate of an accredited therapy program, licensed in the state of practice, and meeting 3 C's criteria.
• Experience: Previous rehab SNF experience preferred; at least 1 year of experience required. This is not a CFY position.
