Qualifications

• Licensed Speech-Language Pathologist in Vermont.
• Ability to work effectively in an interdisciplinary team.
• Strong assessment and evaluation skills.
• Ability to develop individualized treatment plans and discharge plans.
• Ability to participate in consultations, teaching, and meetings with residents, families, staff, and healthcare professionals.

Responsibilities

• Provide speech therapy services promoting excellence in rehabilitation care and patient outcomes.
• Promote interdisciplinary cooperation and support geriatric rehabilitation.
• Evaluate patients timely per state, payer, and facility requirements.
• Screen patients upon admission or when requested by the Director.
• Develop individualized treatment plans and goals based on evaluations.
• Provide treatment per the plan of care and document accordingly.
• Collaborate with other disciplines and staff as per the plan.
• Document and submit treatment charges per policies and procedures.
• Participate in family conferences when requested.

Responsibilities

• Provide speech therapy services that promote excellence in rehabilitation care and quality patient outcomes.
• Promote interdisciplinary cooperation and support the facility's geriatric rehabilitation program.
• Evaluate patients in a timely manner according to state, payer, and facility requirements.
• Screen patients upon admission and/or when requested by the Director.
• Develop individualized treatment plans and appropriate goals based on patient evaluations.
• Provide treatment according to the established plan of care.
• Communicate and collaborate with other disciplines and staff as indicated by the treatment plan.
• Provide routine documentation in accordance with Medicare, facility, and other payer guidelines.
• Participate in family conferences when requested.
• Record and submit all required documentation and treatment charges in a timely manner according to facility policies and procedures.

Qualifications

• Current Vermont State License as a Speech-Language Pathologist.
• Ability to work effectively as part of a dynamic and cooperative interdisciplinary team.
• Strong assessment and evaluation skills.
• Ability to develop individualized treatment plans and appropriate discharge plans.
• Ability to participate in consultations, teaching, and meetings with residents, families, staff, and other healthcare professionals.
