Job summary

Berlin Health & Rehabilitation Center is seeking a full-time Speech-Language Pathologist to join our rehabilitation team. Our focus is on helping residents achieve independence and quality of life through personalized care.

You will evaluate residents, develop and implement treatment plans, provide direct therapy, and collaborate with physicians, nurses, and therapists in a multidisciplinary setting. We offer a sign-on bonus and competitive pay to join our caring community.

Qualifications

  • Master's degree in Speech-Language Pathology from an accredited college or university.
  • Current state licensure as a Speech-Language Pathologist.
  • CCC-SLP certification preferred.
  • Experience in skilled nursing, rehabilitation, long-term care, or geriatric care preferred, but not required.
  • Strong communication, clinical, and interpersonal skills.
  • Ability to work effectively within an interdisciplinary team.
  • Commitment to compassionate, resident-centered care.

Responsibilities

  • Evaluate residents referred for speech-language pathology services.
  • Assess speech, language, communication, cognitive, and swallowing disorders.
  • Develop individualized treatment plans based on each resident's needs, abilities, and rehabilitation goals.
  • Provide direct therapy to improve communication, cognition, and swallowing function.
  • Establish measurable and clinically appropriate treatment goals.
  • Monitor resident progress and modify treatment plans as needed.
  • Complete evaluations, progress notes, recertifications, and discharge documentation accurately and on time.
  • Educate residents, families, and caregivers on treatment plans and therapeutic techniques.
  • Collaborate with physicians, nurses, physical therapists, occupational therapists, and other members of the interdisciplinary team.
  • Maintain accurate and complete clinical documentation.
  • Follow all applicable federal, state, and facility regulations, including HIPAA and resident rights requirements.
  • Perform other duties as assigned by the Director of Rehabilitation.

Responsibilities
