Job summary

Ultimate Therapy is seeking a full-time Speech-Language Pathologist to join Autumn Lake at Crystal Springs in Elkins, West Virginia. You will evaluate and treat adults with speech, language, cognition, and swallowing needs in a skilled nursing setting.

You will set measurable goals, document outcomes, and collaborate with nursing, dietary staff and the rehabilitation team to support residents' communication and daily living.

Qualifications

• Active West Virginia Speech-Language Pathologist license.
• Strong assessment and treatment skills in adult communication, cognition, and swallowing.
• Clear communication and accurate, timely documentation.
• Skilled nursing or geriatric rehabilitation experience is a plus.

Responsibilities

• Evaluate speech, language, cognitive-communication, and swallowing abilities to establish treatment priorities.
• Develop measurable goals that reflect residents' clinical needs and personal preferences.
• Provide individualized therapy for communication, memory, attention, and functional problem-solving.
• Deliver swallowing interventions and coordinate recommendations with nursing and dietary staff.
• Teach residents, families, and caregivers practical strategies to reinforce progress outside therapy.
• Reassess outcomes, maintain clinical documentation, and contribute to interdisciplinary care and discharge planning.

What You'll Do
