Job summary

Resolve Diamondback is seeking a compassionate Speech Language Pathologist to join our rehabilitation team at a skilled nursing facility. You will evaluate and treat communication, cognition, and swallowing disorders to improve residents' quality of life.

Join an experienced interdisciplinary team, provide evidence-based interventions, collaborate with physicians and families, and participate in care and discharge planning while maintaining thorough documentation.

Qualifications

• Master’s degree in Speech Language Pathology is required.
• CCC-SLP certification is preferred.
• Experience in skilled nursing, rehabilitation, or post-acute settings preferred.

Responsibilities

• Provide speech therapy evaluations and develop individualized treatment plans.
• Assess and treat speech, language, cognitive, communication, and swallowing disorders.
• Implement evidence-based treatment interventions to maximize resident outcomes.
• Collaborate with physicians, nursing staff, therapy team members, residents, and families.
• Maintain timely and accurate documentation in accordance with Medicare, state, and facility guidelines.
• Participate in care planning meetings and discharge planning activities.
• Educate residents, families, and staff regarding treatment plans and recommendations.
