Emerald Nursing & Rehab Cozad is seeking a Per Diem Speech-Language Pathologist to join our multidisciplinary team in a skilled nursing facility. You will assess residents, create individualized treatment plans, and provide evidence-based therapy to improve communication and swallowing outcomes.

The role emphasizes collaboration with an interdisciplinary team, ongoing documentation, and a commitment to quality resident care. Prior experience with older adults is preferred.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program (CCC-SLP).
• Current state licensure as a Speech-Language Pathologist (CCC-SLP).
• Experience with older adults and communication/swallowing disorders.
• Ability to work independently and as part of a multidisciplinary team.
• Excellent communication and documentation skills.

Responsibilities

• Conduct comprehensive assessments of residents' speech, language, and swallowing abilities.
• Develop and implement individualized treatment plans with the interdisciplinary team.
• Provide evidence-based SLP interventions, including individual and group sessions.
• Monitor progress and adjust plans as needed.
• Maintain clear documentation and communicate with residents, families, and staff.
• Collaborate with other healthcare professionals to ensure coordinated care.
