Job summary

Meadows Post Acute in Los Angeles, CA, seeks a licensed Speech Language Pathologist to evaluate and treat residents with communication and swallowing disorders to improve quality of life and functional abilities.

You will work with an interdisciplinary team to develop individualized treatment plans, document progress, educate residents and families, and maintain compliance with practice standards in a skilled nursing facility.

Qualifications

  • Current license to practice as a Speech Language Pathologist in the state of employment.
  • Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility.
  • Knowledge of current speech therapy practice standards and regulations.
  • Proficient in using electronic medical records and other healthcare software.

Responsibilities

  • Evaluate residents' communication and swallowing abilities using standardized assessments.
  • Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders.
  • Provide skilled speech therapy interventions, including articulation and language therapy, cognitive-communication therapy, and swallowing therapy.
  • Monitor and document resident progress and adjust treatment plans as necessary.
  • Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents.
  • Educate residents and family members on speech therapy goals, treatment plans, and home exercise programs.
  • Maintain accurate and timely documentation of speech therapy services provided.
  • Participate in team meetings, in-service education, and quality improvement initiatives.
  • Maintain current knowledge of speech therapy practice and relevant regulations.

  • Participate in team meetings, in-service education, and quality improvement initiatives
  • Maintain current knowledge of speech therapy practice and relevant regulations
