Job summary

Therapy 2000 in Houston is seeking a licensed Speech-Language Pathologist to provide home-based pediatric therapy to children in the Central Southwest Almeda area. This role emphasizes clinical excellence, collaboration, and delivering high-quality care in the child’s natural environment.

A sign-on bonus is included. You will evaluate, treat, and coordinate with families and other providers, maintain charts, complete visits, and participate in ongoing CE opportunities.

Qualifications

• Current/active license to practice Speech Language Pathology in Texas.
• 3+ years as a pediatric Speech Language Pathologist.
• Experience in a home-health setting.
• Experience or certifications in feeding therapy or related specialties.

Responsibilities

• Perform comprehensive Speech Therapy evaluations using standardized tests to assess needs.
• Develop and implement treatment plans aligned with physicians’ orders.
• Create home exercise programs in partnership with caregivers.
• Collaborate with other providers to coordinate care for patients.
• Complete minimum patient visits weekly as required by the agency.
• Maintain patient charts and submit progress notes and logs on time.
• Meet with agency staff to align on expectations and standards for care.
• Adhere to agency policies, procedures, and applicable laws.

Qualifications

What you’ll need:

• A current/active license to practice Speech Language Pathology in the state of Texas.

Responsibilities

What you’ll do:

• Perform comprehensive Speech Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physicians’ orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric speech language pathology.
