Job summary

Autumn Creek Post Acute is seeking a Speech-Language Pathologist (CFY) to provide direct patient care and support the therapy department in California.

You will evaluate patients, create and adjust treatment plans, educate families, and document services in medical records while collaborating with physicians and the interdisciplinary team. Supervisory duties include Clinical Fellows and other SLPs as needed.

Qualifications

• Master’s Degree in Speech-Language Pathology from an accredited program.
• Experience in a skilled nursing rehab setting preferred; required.
• CCC-SLP and California license (or license candidate) required or pursuing.

Responsibilities

• Evaluates and treats patients to maximize speech, language, cognitive and swallowing abilities.
• Develops treatment plans and obtains physician approvals as needed.
• Documents services in medical records; ensures accurate billing data.
• Educates patients’ families and nursing staff on follow-through programs.
• Communicates with physicians and the interdisciplinary team regarding progress.
• Supervises Clinical Fellow and other SLPs in patient care activities.
