Job summary

Focused Post Acute Care Partners FPACP in Huntsville, TX seeks a Speech Language Pathologist to evaluate patients and develop therapy programs in collaboration with physicians. You will provide therapy, guide patients toward goals, and coordinate with interdisciplinary teams to ensure quality care.

The role requires a Master's Degree in Speech Language Pathology and current state licensure, with the Rehab Director and team supporting your ongoing professional performance.

Qualifications

• Master's degree in Speech Language Pathology and state licensure required.
• Clinical training in Speech Language Pathology and licensure as SLP.

Responsibilities

• Evaluate patients and develop speech language programs in collaboration with physician.
• Instruct, motivate and assist patients in reaching therapy goals.
• Establish and evaluate treatment plans, goals and discharge plans.
• Coordinate therapy programs with other health care providers.
• Maintain timely and accurate clinical records and documentation.
• Comply with Medicare guidelines and obtain prior approvals as required.
• Schedule patients and communicate progress with team and families.
